This notice describes how health information about you may be used and disclosed, and how you can get access to this information. It applies to information we create or hold in the course of providing home care services to you.

Our duty

We are required to keep information about your health private, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose information about you

For your care

We share information with the caregivers assigned to you and with the supervisors overseeing your service plan, so that the care you receive is the care you agreed to.

For payment

We use information to bill you or a third party responsible for payment, and to answer questions about a bill.

For our operations

We use information to run the agency: supervision, quality review, training, and complaint investigation.

Where the law requires it

  • Reporting suspected abuse, neglect, or exploitation.
  • Responding to the Pennsylvania Department of Health, Division of Home Health in the course of licensure inspection or complaint investigation.
  • Responding to a court order, subpoena, or other lawful process.
  • Preventing a serious and imminent threat to health or safety.

To people involved in your care

We may share relevant information with a family member, a legal representative, or another person you identify. You can ask us to limit this, and we will honour reasonable requests.

Uses that always require your written authorisation

Marketing, any sale of information, and most uses of psychotherapy notes require your written authorisation. You can revoke an authorisation at any time in writing, which stops future uses.

Your rights

  • Inspect and copy your recordYou can ask to see and get a copy of the information we hold about you.
  • Ask us to correct itIf you believe something is wrong or incomplete, you can ask us to amend it.
  • Get a list of disclosuresYou can ask for an accounting of certain disclosures we have made.
  • Ask us to restrict what we shareYou can request a limit on what we use or disclose. We will accommodate reasonable requests.
  • Ask us to contact you a particular wayBy a specific phone number or address, for example. Reasonable requests will be accommodated.
  • Get a paper copy of this noticeAt any time, on request, even if you agreed to receive it electronically.
  • Be notified of a breachIf your unsecured information is breached, we will tell you.

Changes to this notice

We may change this notice and make the new terms apply to information we already hold as well as to information we receive in future. The current notice is always posted on this page.
